INFORMATION FOR NEXT OF KIN
The following information, which you should organize and update on a regular basis, will greatly assist the people you wish to be responsible for your affairs in the event of your incapacity or death. Keep the information in a specific location that is obvious or is known to them. One suggestion is a Red, three ring binder. Not only will this be easy to find, but in case of emergency, the binder can "go along" to provide important information!

1.Your full name, date of birth, social security number, California driver's license number or DMV identification number.

2.Dated list of names, addresses, telephone numbers of your doctor, dentist, attorney, accountant, financial planner, broker, insurance agent, minister.

3.A copy of your estate planning documents, including your Durable Power of Attorney for Health Care. Indicate the location of the original estate planning documents, deeds, insurance policies, etc.

4.A dated list of assets.

5.Dated list of insurance policies (life, health, assets and liability, etc.)

6.Information regarding retirement benefits, any expected death benefits (social security, employee benefits, accidental death benefits, incidental life insurance policies associated with credit union accounts or credit cards, etc.)

7.Location of safe deposit box, and dated list of contents.

8.List and location of important papers not in safe deposit box.

9.Dated list of credit card numbers.

10.Dated list of debts, mortgages, obligations.

11.Dated list of subscriptions and memberships.
Leave with a trusted neighbor or friend the name, address, and telephone number of next of kin, and if you wish, information about your medical insurance coverage. A good place to keep one of your Health Care Power of Attorney Identification Cards is on the refrigerator.
